For a smaller town's landline area code, a quick search of the city name plus "área código Colombia" gives you the right prefix.

Calling a Bogotá landline whose local number is 234 5678: dial 011 (exit) + 57 (Colombia) + 1 (Bogotá) + 234 5678. On a cell phone: +57 1 234 5678.

Calling a Colombian mobile number like 300 123 4567: dial 011 (exit) + 57 (Colombia) + 300 123 4567, with no area code inserted. On a cell phone: +57 300 123 4567.

Notice the mobile number already starts with a 3 and needs nothing extra between the country code and the 10 digits — adding an area code there is the single most common mistake.
